Incapacitated executor after grant issued

Hi all
I am dealing with an estate where the executrix (she is one of 3) has lost mental capacity and there are assets still to encash. Can someone tell me the procedure for revoking the current grant and obtaining one in the names of the two capable executors, with power reserved to the incapacitated executrix?

I did similar application in 2020 with affidavit for order under rule 41 of the Non-Contentious Probate Rules 1987

Not sure if can do now via statement of truth?

Just had a look remotely at what I sent in 2020, appear to be also PA1P, IHT205
